Your Job:

Provides direct patient care through case management, program development and delivery, assessment and documentation of patient’s needs.  This position requires that the employee demonstrate the knowledge, skills, understanding and ability to care for patients of the adult and older adult (geriatric) age groups and be sensitive to the diverse and unique needs of patients based on age, sex, race, and culture.

Your Job Requirements:

• Formal training, certification and/or licensure in area of specialization (including Licensed Chemical Dependency Counselor, and evidence of current licensure in the State of Texas.
• Minimum of two years experience under licensure or certification of area of specialization.
• CPR “C” certified.
• Annual training in Management of Aggressive Patients.

* Facilitate Chemical Dependency group therapy 3 days weekly.

Your Job Responsibilities:  

• Provides case management and documentation of these activities to include: discharge planning treatment planning, family and significant other contact when indicated, and contact with agencies, etc. in coordinating appropriate continuing care.
• Coordinates clinical program, facilitates and develops new programs, documents group notes and other pertinent information.
• Completes assessment of the patient in their area of expertise including chemical dependency assessment, diagnostic summary, and/or activities assessment.
• Participates in CQI projects or other activities as directed by supervisor.
• Provides information to payors for continued stay reviews of chemically dependent outpatients.
