About Us – Social Services

• The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc

• Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho

• Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible

What You’ll Do

Establish positive and professional rapport with patients, their families and caregivers and provide assistance in accessing identified resources.

Research available resources and establish relationships with community providers to better serve patients and their families.

Assist the clinic or community team with development of care plan. Provide support to patients and families to achieve the identified goals of the care plan.

Requirements

Experience gained through HS diploma.

Experience working in a community based setting/organization for 1-3 years.
