23.64 to 25.85 USD Per Hour

Working Hours/Shift:

Monday - Friday (8-5pm Flexibility Available)

What you bring to SBHG (Qualifications):

Education & Experience: One of the following combinations is required:

  • Master’s degree in social science plus one (1) year of experience in behavioral health.
  • Bachelor’s degree in social science plus two (2) years of experience in the behavioral health field.
  • Associate’s degree in social science plus three (3) years of experience in the behavioral health field.

Licensing: Valid California Driver's License.

 Driving Record: Must meet Agency driver eligibility criteria.

How you will make a difference (Job Overview):

This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the Full Circle Health ECM qualifying youth.

Division/Program Overview:

An ECM Manager is a Lead Care Manager specializing in Enhanced Care Management, providing comprehensive support to enrolled individuals. They act as a central point of coordination, connecting members with vital resources such as housing, food, and childcare, while also collaborating with diverse healthcare providers to ensure seamless and coordinated care.
