  • Salary: $112,396- $116,038 p.a. + Super + Salary packaging

About the role

  • Provide comprehensive physiotherapy assessments and deliver evidence-based interventions to clients with chronic and complex health conditions.

  • Develop and implement tailored care plans that promote functional independence, improve quality of life, and support long-term condition management.

  • Collaborate closely with multidisciplinary teams, community services, and healthcare providers to ensure coordinated, person-centred care and appropriate referrals.

  • Expand your clinical expertise by working with a diverse caseload and contributing to ongoing service development and research initiatives.

  • Be part of a supportive and collaborative Chronic and Complex Care team committed to professional growth, mentorship, and delivering exceptional care to our community.


About you

  • You’re a qualified and AHPRA-registered Physiotherapist, with a current Class C driver’s license.

  • You bring proven experience in community settings and have a passion for delivering compassionate care to our vulnerable populations.

  • Confident in applying clinical reasoning and exercising sound professional judgment in a fast-paced, multidisciplinary environment

  • A natural communicator who thrives in a team, building strong relationships with patients, families, and colleagues alike

  • Committed to continuous improvement, with a demonstrated history of supervising junior staff, students and contributing to quality initiatives through evidence-based practice.
