Utilizing a collaborative effort, provides assessment, planning and treatment for patients in a primary care setting to address mental health, substance use and other health behaviors that may contribute to chronic illnesses. Coordinates with physicians, interdisciplinary staff and family/significant others as required or requested by the patient. Coordinates a range of services as needed to provide individualized and Person-Centered Treatment and complete all associated documentation in a timely manner. Maintains current licensing/certifications and credentials and maintains knowledge of current trends and developments in the field. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area. This position has a matrix reporting system to the HHMG Practice Manager and Behavioral Health Service Leadership.

Pay Range:  $29.96 - $49.25/hour

Employment Type:  Part Time, 40 hours every 2 weeks

Schedule:  Varies

Requirements:  Masters Degree in Social Work required, Fully licensed Social Worker in the State of Michigan (LMSW) required

Assessment and Treatment

  • Provides accurate and thorough individualized assessments, treatment planning and individual, group and family therapy.
  • DSM diagnoses are consistent with the presenting problem, history of problem, mental status exam and other assessment data.
  • Reviews and modifies the treatment plan as needed and appropriate.
  • Collaborates with HHMG/BHS colleagues, outside care providers, family and others as needed to implement a successful and individualized plan of care.
  • Provides treatment that is focused and effective.
  • Coordinate care with community providers and facilitate referrals as appropriate.

Clinical Documentation/Communication

  • Documents assessment, education and treatment in a timely, accurate manner that is in compliance with regulatory requirements, policies and procedures.
  • Develops open collaborative communication with the care team.
  • Attends huddles and care conferences as appropriate.

Quality Improvement

  • Participates in problem identification and continuous quality improvement related to clinical care, regulatory compliance and coordination of care.
  • Maintains knowledge of current trends and developments in the field.
  • Utilizes supervision and consultation appropriately.
  • Understanding of the payor data and HHMG registry.
