Job Description Summary:
Responsible for planning and directing the delivery of hospital care as it relates to the care of adult patients who are critically/acutely ill in the intensive care medical and surgical units. Employee will provide critical/acute care and health education for adult and geriatric patients. Employee will work under the supervision of the Program Medical Director, the Intensivist Team and the guidance of established protocols as required. Employee shall enter into collaborative agreements with one or more physicians as directed by the employer. They will contribute to accomplishment and support of the mission, vision, and values of the Jackson-Madison County General Hospital. Models appropriate behavior to employees as exemplified in the Quality Plus manner, which reflects commitment to the Continuous Process Improvement Plan (CPI). Responsible for insuring all core measures are met. This position reports directly to the Intensivists Program Medical Director. Responsible for the assigned shift as scheduled and is subject to call back as required by WTH. Employee understands they are being hired to work on a full time basis generally consisting primarily of fourteen (14) night shifts per month. Shift coverage may be altered as determined by the Intensivist Program Director and Intensivist Team to meet future patient care needs. Shifts are also subject to non-traditional hours, in an exempt status.
ESSENTIAL JOB FUNCTIONS:
- Collaborates with the intensivist in establishing/reviewing protocols for managing critical, acute, and long-term medical needs of patients
- Performs a complete physical exam, obtains a complete medical history and records
- findings. Orders appropriate laboratory and diagnostic procedures. Interprets and integrates data to determine preliminary diagnoses and therapeutic plan.
- Provides medical services and health education to adult and geriatric patients under the
- direction of the intensivists.
- Performs necessary diagnostic and therapeutic procedures according to approved protocols developed with and under the supervision of the intensivists which may include the following but are not limited to the following: securing, recording and evaluating the health psychosocial and developmental history of patients, ordering laboratory and x-ray studies, carrying out prescription drug orders under protocols, management of common or chronic illnesses, referrals, etc. Employee documents the process of critical and acute care delivery and procedures.
- Administers or gives orders to nursing support to administer medications and injections in compliance with state law and hospital guidelines.
- Functions as a patient advocate, conducts follow-up care and coordinates care with other healthcare providers and makes appropriate referrals for continuity of care.
- Maintains accurate and complete medical records in a timely fashion.
- Demonstrates effective AIDET skills with patients, families, and guests.
- Provides education counseling that will enhance patient care and preventative knowledge.
- Contributes to a positive work climate and overall team effort of the intensivist service, departments, and hospital.
- Responsible for supporting staff in providing quality care and promoting quality outcomes.
- Supports and guides a working environment that stays current with research and guidelines in critical and acute care medicine.
- Performs related responsibilities as required or directed and remain on-site until employee’s assigned patient care duties have been completed.
- Be able to multi-task and prioritize workload in a fast-paced environment, lift and move patients as necessary, and work on a computer for sustained periods of time.
- Practice Employee’s profession using evidence-based practices; standardized protocols and order sets; appropriate utilization of resources; and coordinator of care to achieve the most appropriate length of stay.
- Daily planning and coordinating care with all members of the health care team to assure maximum quality and efficiency, including daily coordination with the multi-disciplinary care team, including other team members, respiratory therapy, pharmacy, nursing and various other departments of the hospital.
- Perform such other administrative and clinical duties related to the delivery of services as may be reasonably requested by the Intensivists Service from time to time.
JOB SPECIFICATIONS:
EDUCATION:
- BSN and a Master’s Degree in Nursing with successful completion of a Nurse Practitioner Specialty or Physician Assistant Program as approved by the state of Tennessee
- Specialty training/certification in Critical/Acute Care or equivalent experience in an Acute/Critical Care Unit preferred.
LICENSURE, REGISTRATION, CERTIFICATION:
- RN license, APN license and certification for ACNP or NCCPA certification for PA
- Current DEA License issued by the Drug Enforcement Agency
- Current ACLS certification
- Responsible for Continuing Education units annually
- Fundamental Critical Care Support Certification preferred
EXPERIENCE:
- Prefer experience in a Critical/Acute Care setting within the past two years
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of nurse practitioner principles or PA principles and techniques in a Critical/Acute care setting
- Demonstrate the ability to work cooperatively and productively in a team environment with a diverse set of employees, patients and families.
- Process computer skills sufficient to conduct word processing for document development and patient record documentation.
- Must successfully complete a period of proctoring and training as prescribed by the Department of Hospitalists/Intensivists Program Director and established protocols.
