The Division of Comprehensive Care at HDVCH is recruiting a Pediatric Complex Care APP.   This APP will help develop the new program to care for children with medical complexities along with the pediatric complex care physicians.

The complex care APP will be responsible for providing care to an appropriate cohort of medically complex pediatric patients including:

  • Evaluation and management common clinical issues in children with medical complexity

  • Provide comprehensive perioperative assessment and management

  • Develop and implement safety and emergency plans

  • Develop and manage enteral nutrition plans for children with enteral nutrition dependence

  • Perform effective, team-based, patient-and family centered care coordination, advocate for children with medical complexity and their families.

  • Provides input into the development of policies and procedures as they related to medical practice and quality patient care.

  • Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.

This provider will also importantly collaborate with the pediatric subspecialists to provide a comprehensive overview and engage specialists as needed. In addition, the new APP will collaborate with the physician to provide support to the Care Partners nursing teams who provide care coordination and navigation to a cohort of medically complex patients.

Qualifications: 

Nurse Practitioner: 

  • Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission 

  • 3 years of relevant experience current, relevant clinical experience 

  • Previous experience functioning in a collaborative role as a Nurse Practitioner 

  • LIC-Nurse Practitioner (NP) - State of Michigan  

  • CRT-Basic Life Support (BLS)  

 Physician Assistant: 

  • Required Master's Degree Graduate of an accredited Physician Assistant educational program 

  • LIC-Physician Assistant - State of Michigan 

  • CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants  

  • CRT-Basic Life Support (BLS)
