Job Summary
The Community Health Worker (CHW) Intern participates in a six (6) month rotational program within the Community Health departments to provide exposure and build awareness to optimal community resources that are tailored to patient needs. This position engages in community health department rotations, completing assigned projects under the guidance of the supporting manager to gain exposure to healthcare, organizational practices, and community health resources. The CHW Intern will learn to utilize community resources and establish connections with providers to identify suitable medical, mental health, and dental homes for uninsured and under-insured populations. The CHW intern will rotate and travel across Memorial Hermann locations within the greater Houston area. The rotation schedules, program terms, and assignments are subject to change based on program availability and organizational needs. This role typically reports to a Manager within the Community Health/Community Benefits department.MINIMUM QUALIFICATIONS
Education: High School Diploma or GED preferred.
Licenses/Certifications: Certified Community Health Worker preferred.
