Position Summary: Responsible for patient flow activities throughout the organization including internal and external transfers, direct admits, and Emergency Department services. Responsible for coordinating care with the ED and primary physician team assignments and the availability of beds for primary and specialty care services in the Medical Center. Promotes effective flow of patients throughout the Medical Center. Attend daily care coordination rounds; strategize and recommend anticipated placement of patient to appropriate unit at an appropriate time. Collect, analyze, evaluate and summarize data from referring hospital.

Position Requirements: Bachelor's degree in nursing or advanced nursing degree. Licensed to practice as a Professional Registered Nurse in the State of Georgia or have an active Multi-State Registered Nurse Compact License. Current American Heart Association or American Red Cross Basic Life Support (BLS). 5 years or more of experience in beside nursing.

Preferred Requirements: Master's Degree in Nursing or MSHA w/ BSN. 2 years or more of experience in ICU/ER setting, Care Coordination, and Utilization Review.

Scheduling Requirements: Every other weekend
