Full-Time | Salaried | Monday-Friday, 8:00 AM - 5:00 PM

Your Impact as a Behavioral Health RN Case Manager:

  • Patient Care Management: Take ownership of a caseload of behavioral health patients, assessing their overall health and psychosocial needs, and ensuring they receive the care they need.

  • Plan Development: Collaborate with the interdisciplinary team to create personalized care plans, adjusting as necessary to address changing patient needs.

  • Communication & Collaboration: Work closely with physicians, clinical supervisors, and healthcare team members to ensure timely interventions and excellent patient care.

  • Education & Support: Provide valuable education and support to patients, families, and facility staff, ensuring the best possible care at all times.

  • Leadership: Supervise and guide behavioral health aides and LPNs, ensuring consistent, high-quality care.

What You’ll Bring:

  • Current RN License (unrestricted in the state)

  • 1+ year of clinical experience (Behavioral Health, Home Health, or Hospice preferred)

  • Reliable Transportation: A dependable vehicle, valid driver’s license, and auto insurance are required, as you'll travel within the coverage area.

  • 50% Travel Required: Be ready and willing to visit patients across the service region.

  • Educational Background: Associate's or Bachelor's Degree in Nursing (preferred).
