Position Summary

PACE Primary Care Providers (PCPs) deliver comprehensive, patient-centered primary care to a medically complex, frail elderly population. This role includes managing chronic diseases, addressing acute care needs, providing preventive services, and overseeing medication management.

PCPs work closely with the interdisciplinary team (IDT) to coordinate care, develop individualized care plans, and ensure timely documentation and follow-up based on biannual and as-needed assessments.

In addition to clinical responsibilities, PCPs actively contribute to quality improvement initiatives aimed at:

Reducing hospitalizations and emergency department utilization

Improving risk adjustment accuracy

Closing preventive care gaps

Enhancing chronic disease outcomes



Key Responsibilities

Provide high-quality, compassionate primary care to participants and their families

Maintain accurate and timely documentation that meets legal and clinical standards

Conduct goals-of-care discussions with patients and families

Collaborate effectively with the interdisciplinary team and other Hope Healthcare services

Participate in quality improvement and performance initiatives

Provide on-call coverage for afterhours/holidays/weekends (rotation basis, if applicable)



Knowledge & Skills

Expertise in Adult Health, Primary Care, and/or Geriatrics

Strong understanding of interdisciplinary team-based care

Ability to manage complex, high-risk patients

Adherence to the American Nurses Association (ANA) standards, including:

Scope and Standards of Practice

Code of Ethics for Nursing

Nursing’s Social Policy Statement

Specialty Nursing Standards



Required Qualifications

Master’s degree in nursing from an accredited program

Specialty in Adult Health, Primary Care, or Geriatrics preferred


Team-based model focused on quality outcomes and patient impact

Certifications/Licenses/Registration:

Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation.
A current license to practice Advanced Nursing in the State of Florida is required. Credentialing through ANCC as a NP is preferred. Certification in Hospice and Palliative nursing is preferred.

Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Experience Requirements

Knowledge of the principles and practices of primary medical care and palliative medicine. Certification in Hospice and Palliative nursing is preferred.
Position Specific Responsibilities (not all inclusive)
Provide medical care for patients assigned in person, and by telephone communication with staff nurses.

Provide documentation in the patient record that meets criteria for good medical practice, addresses legal mandates, and adequately supports billing codes.

Communicate successfully with other Hope HealthCare Services employees, the public, and community physicians in a manner that represents Hope’s culture of a collaborative and positive work environment.

Assist the Medical Director in maintaining 24 hour, 7 day per week medical coverage for PACE patients in Hope Hospice facilities and services (if applicable).

Provide on-call coverage for Hope Palliative Care patients/families on a rotation basis (if applicable).

Participate in Hope PACE medical staff meetings and complete any mandatory continuing medical education activities as directed by the administration, in addition to CME needed to maintain certification.

Participate in any Quality Assurance activities approved by the administration as appropriate to your job setting.

Follow federal and state laws regarding medication prescription.
Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.

Consistently represents Hope with integrity and professionalism.

Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.

When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.

Assist with special projects, as requested.

Compensation Pay Range:

$88,500.00 - $134,300.00
