This is a fully remote role, working with the CVS Accountable Care Mt. Sinai team. Hours for this position are Monday-Friday <8:30am-5pm>.

The Post Acute Care Coordinator is responsible to facilitate patient referrals for ACO patients to appropriate community services, including home care, and to ensure follow-up on these referrals while providing status updates to all members of the care team. The Post Acute Care Coordinator will also address any barriers that may impede the timely initiation of these services and accurately document interventions in the Electronic Medical Record (EMR).

Additionally, Care Coordinator will support Post-Acute Care team members by assisting with the scheduling, modification, and cancellation of appointments with physicians to facilitate the timely discharge of patients from skilled nursing facilities. The Post Acute Care Coordinator will report directly to the Associate Director of Care Continuum.

Responsibilities:

1. Identifying and formulating a daily list of ACO patients for Home Care intervention

2. Referring patients to appropriate community services including home care as instructed by Home Health RNs/PTs/Associate Director

3. Following up on the referral and resolving barriers that delay the initiation of services

4. Closing the loop on the referral and providing an update to all parties involved.

5. Effectively coordinating and communicating with patients, providers, internal stakeholders, and external vendors

6. Clearly and timely documenting in the EMR

7. Assisting the Post-Acute Care team members with scheduling, modifying or cancelling appointments with physicians to help expedite patient discharge from skilled nursing facilities

8. Performing other duties as instructed by Home Health RNs/PTs/OTs, Associate Director, Vice President and other CVS ACO and/or MSHP leadership

Required Qualifications

• 1-3 years relevant experience in health care, patient navigation or related field preferred

• Ability to learn hospital documentation and scheduling systems and care coordination applications

Preferred Qualifications
• Bachelor’s degree, preferably in a health-related profession or combination of applicable experience and education

• Call center experience

• Computer proficient with outlook, windows, and word

• Excellent oral and written communication skills and organizational skills
• Experience collaborating with medical professionals and prior use of electronic medical records.

• Ability to foster a supportive environment for beneficiaries and work collaboratively with a multi-disciplinary team

• Organized – detail-oriented; consistently meets and tracks timelines and objective

Additional requirements for this position include a general medical exam, proof of vaccinations or past disease, or ability to obtain necessary vaccinations.
 

Pay Range

The typical pay range for this role is:

$18.50 - $38.82

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
