As a Case Management Assistant, you will facilitate the transition of patients to post-acute levels of care identified by the Transitional Care Coordinator (TCC) and partner with TCC and/or Social Workers (SW) to complete the important message process and to assist with procurement of supplies needed to fulfill a patients discharge plan. You will also be responsible for communicating with extended care facilities regarding bed availability, ensuring that patients are transitioned to lower levels of care that are appropriately covered by patient's 3rd party payer and that necessary clinical documentation is supplied to providers, arranging transportation, investigating resources for medications, medical supplies and other medical devices as needed, and documenting details of the discharge transition plan. 
 

A caregiver in this position works 9:00AM – 5:30PM.  


A caregiver who excels in this role will:  

  • Participate in discharge planning activities for complex patients and partner with Transitional Care Coordinators and social workers to ensure a timely discharge to provide appropriate linkage with post-discharge providers. 

  • Communicate with Transitional Care Coordinators and social workers regarding the discharge planning status of all patients referred by Transitional Care Coordinator. 

  • Determine bed availability at extended care facilities based upon requests by patients and families to transfer to selected facilities and then advise the Transitional Care Coordinator and social workers of bed opening. 

  • Transmit initial patient demographic information as appropriate. 

  • Coordinate paperwork for PASS-ID and all other regulatory documents that may be required prior to transition of patients to post-acute care. 

  • Prepare necessary information from the medical record prior to transition of patients to post-acute settings. 

  • Arrange for transportation to post-acute care and document arrangements in the medical record. 

  • Maintain ongoing communication with Transitional Care Coordinators and social workers of the status of transition plans assigned to CMA. 

  • Assist with arranging for procurement of difficult to find medications, medical supplies and other items that will be needed to fulfill a patient's discharge plan. 


Minimum qualifications for the ideal future caregiver include:  

  • High school diploma or GED. 

  • Three years of recent experience working in healthcare or a related field.  

  • Fluent in medical terminology.  

  • Computer literacy.
