Job Description:
This position works 40 hours a week, mostly work from home with a few days a year required for in-person meetings. Candidates must be in the Denver/Colorado Springs area to be considered.
The Outreach Referral Coordinator supports member outreach and engagement operations, with the goal of connecting members to existing health services to effectively navigate and maximize their use of the health system. This includes coordinating referrals to/from our provider and community-based organization network.
Essential Functions:
- Liaises between Health First Colorado members, health systems, medical providers and the CCHA Member Support Services call center in order to navigate the medical home system of care
- Acts as subject matter expert for Outreach Referral Coordinator initiatives to accurately coordinate referrals to the Care Coordination team and meets required turnaround times and documentation standards
- Effectively navigates systems while developing strong telephonic relationships with members to include educating members on proper use of the health care system, completing health risk assessments, providing and/or arranging appropriate referrals to medical and non-medical services, and completing timely follow-up
- Supports Emergency Department Follow-up Outreach and Transitions of Care program by processing daily census data, creating and assigning care coordination cases, with accuracy
- Completes outreach requirements and inbound queue call duties as assigned
- Handles escalated members and/or members with high acuity and high intensity needs
- Provides feedback and recommendations to leadership regarding duties and functions to streamline roles and responsibilities
- Participates in meetings and presents information as needed
- Reports on outcomes related to Outreach and Referral Coordinator initiatives
- Maintains confidentiality and ensures compliance with HIPAA regulations

Qualifications:
- High school diploma or GED
- Experience working in a health care setting (Medical Assistant-preferred)
- Experience triaging inbound referrals and coordinating outbound referrals
- Community Health Worker/Care Navigation experience (preferred)
- 3-5 years’ experience in customer service OR related equivalent experience
- Bilingual; fluent in English and Spanish (preferred)
- Home office that is HIPAA compliant for all remote or telecommuting positions as outlined by the company policies and procedures
Salary Range:
$17.45-$23.27
