JOB DESCRIPTION:

Monday-Friday

9am-4pm

SUMMARY OF POSITION

Leads the coordination for the treatment of behavioral health conditions (mental health & substance use) and physical health conditions. Providing comprehensive care management and coordination; transitional care; health promotion to improve overall wellness and manage chronic health conditions; and monitoring health indicators.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Leads the work of subordinate staff.
  • Ensures client`s care is coordinated between client and all healthcare providers.
  • Ensures clients` access to needed services during transitions from different levels of care.
  • Provides education, reinforcement and frequent communication with clients about their physical and behavioral health status.
  • Develops strategies with clients so they can reach their health care goals.
  • Teaches individuals disease management strategies.
  • Provides encouragement and support to clients to assist them in solving difficulties they have managing and improving their health.
  • Develops strategies to remove barriers, allowing clients to achieve optimal health status. 
  • Achieves targets related to participant contact.
  • Ensures provided lunch is ready and food safety policies have been followed.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Experience working with individuals with mental illness is preferred. 
  • Ability to work independently and be part of a team is required
  • Ability to think critically and use data to assist participants is required
  • Belief in the concept of mental health recovery.

QUALIFICATIONS

Education Level              

Bachelor’s degree from a four-year college or university is preferred; preferably in psychology, social work, or related field. 

Years of Experience       

Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree

Certification/Licensure 

Must complete Serv- safe certification within 90 days of hire.

$37,850.00--$53,030.00
