As an RN Care Coordinator, you will work closely with a highly engaged interdisciplinary urogynecology team to support patients with complex and specialty care needs across the continuum of care. This role serves as a primary point of contact for patients, conducting clinical assessments, coordinating care plans, and providing education and outreach to improve outcomes and patient satisfaction. Responsibilities include managing prior authorizations and appeals for procedures, ensuring accurate order placement, performing chart reviews, coordinating surgical clearances, optimizing provider schedules, preparing patients for procedures, triaging clinic patients, and facilitating home care services as needed. You will collaborate with providers, insurance teams, and support services to close care gaps, improve access, and reduce avoidable delays while maintaining a strong focus on patient-centered, high-quality care.
A caregiver in this role will primarily work 8:00am to 4:30pm.
This is a hybrid role. For working days in the office, there is dedicated space at Hillcrest Hospitals. Office space can be arranged if a west location is preferred.
