Job Summary
 

Job Responsibilities

  • Partners with the Onsite Leadership, National Leadership and other members of the Population Health team to ensure that care teams know how to perform cOS outreach and how to access their resources.

  • Manages the training of new team leaders on cOS, outreach workflows, goals & expectations.

  • Audits outreach to ensure that it is up to our XO standards.

  • Provides strategic advice and recommendations to the leadership team in the development, implementation, and evaluation of new or modified cOS policies, practices, and procedures

  • Develops and manages active patient outreach for new and inactive patients at the Onsite and in partnership with the National team

  • Partners with National Leadership and Onsite Leadership Patient on patient retention outreach efforts

  • Liaison between Population Health Program Manager, cOS champions and multidisciplinary team to share updates, education, and cOS related topics 

  • Leads outreach initiatives to engage patients in all regions (Onsite, Nearsites and in partnership with National team)

  • Leads Onsite cOS efforts and is a cOS advocate for client both for the onsite and national teams

  • Partners with Population Health Manager and CSM to work cross functionally to respond to urgent client requests

  • Submits internal team requests and tracks to completion (ex: Business Intelligence/DSA, Tech)

  • Manages, maintains and updates cOS resources 

  • Participates in quality improvement programs to ensure the practice is performing efficiently and in accordance with Crossover’s policies

  • Assists with workflow design, process improvement, quality measurement, and issue resolution 

  • Demonstrates proficiency in the use of healthcare analytic tools, systems, and analysis 

  • Oversees and supports population health programs and clinic outreach initiatives

  • Administrative assistance as needed for Medical Group leaders

  • Clinical support and lead nurse resource for onsite team 

  • Acts as CTC for OS Platform messaging triage and management

  • Contributes to QBR presentations and discussions 

  • Performs other duties as assigned

Required Qualifications

  • Graduate from an accredited school of nursing 

  • Minimum 3 years of clinical experience 

  • Current Registered Nurse license in the state you are practicing

Preferred Qualifications

  • Bachelor of Science Degree in Nursing (BSN) 

  • 1 year of Crossover Health experience as a nurse 

  • Knowledge of Apple and Google-based technology or client technologies such as Microsoft or HP 

  • Passion for delivering a best-in-class patient care experience in an integrated patient-centered primary care model 

  • Creative, flexible and easily adapts to changing priorities 

  • Energetic, takes initiative and able to work in a team environment as well as autonomously when needed 

  • Embraces new ways of delivering healthcare

Physical Job Requirements 

  • Requires standing, walking and sitting for extended periods of time 

  • Includes full range of body motion including potential of handling and lifting patients 

  • Requires corrected vision, hearing and speech within normal ranges
