As the central connection between patients, providers, and care teams, you’ll ensure smooth transitions that support quality outcomes, reduce length of stay, and help patients confidently move to the next step in their care journey.

Job Description

As an ADT RN, you help create a seamless patient experience from arrival to discharge. Your coordination and clinical expertise ensure safe transitions, improved patient outcomes, and a more efficient healthcare environment.

Hours: 16 hours per week - Saturday and Sunday

What You’ll Do

Admissions

Conduct initial patient assessments and verify physician admission orders

Initiate individualized plans of care

Complete medication reconciliation and ensure accurate documentation

Discharges

Finalize discharge orders and ensure all documentation is complete

Provide clear patient and family education for continued care at home

Coordinate follow-up appointments, prescriptions, and resources

Transfers

Facilitate patient movement between units (e.g., ED to ICU) or to external facilities

Deliver detailed hand-off communication to maintain continuity of care

Collaborate closely with receiving teams to ensure safe transitions

Throughput Optimization

Partner with physicians, case management, and bedside teams to streamline patient flow. Identify and resolve barriers to timely admissions, transfers, and discharges. Support efficient bed turnover and optimal unit capacity

Additional Job Description

Education

Graduate from an accredited school of nursing

Licensure & Certifications

Current Registered Nurse license in South Carolina or compact state

Basic Life Support (BLS) certification from the American Heart Association (AHA)
