Job Summary: Reviews documentation in inpatient and/or IVR (interventional radiology) medical records, and accurately and completely assigns appropriate diagnostic and procedural ICD-9-CM and ICD-10-CM/ICD10-PCS or CPT-4 HCPCS codes to the greatest specificity, and assigning the most accurate DRG/APC as appropriate. Abstracts demographic and coding information into the information system accurately and completely. Core Responsibilities and Essential Functions: Codes and abstracts records: - Accurately and completely assigns appropriate diagnosis and procedural ICD-9-CM and ICD-10-CM/ICD10-PCS or CPT/HCPCS codes to the greatest specificity with a minimum 95 %accuracy rate. - Accurately and completely abstracts all required data into the appropriate data fields in compliance with statistical data requirements with a minimum accuracy rate. - Accurately assigns correct APC a minimum accuracy rate. - Meets productivity standards - Queries physicians when needed to further clarify code assignments Manages additional coding responsibilities, contributing to the CFB (candidate for bill) goals, including but not limited to: - Resolves coding edits and reminders, correcting abstracting and coding issues in a timely manner (1-2 business days) - Completes and routes problem accounts, ready to code, high dollar and other accounts daily to ensure coded as close to goal date as possible - Completes assigned work by goal date - Assists with coding unassigned or backlogged accounts Other task as assigned: - Serves as a mentor to new coders - Assist with cleaning up or escalating missing documentation or other work queues - If proficient, assists with observation, same day surgery, outpatient and emergency coding when needed or assigned Required Minimum Education: High school graduate or possess a General Educational Development (GED) Diploma from an accredited program Required and Completed course in anatomy and physiology Required Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • American Health Info Mgt Cert-Preferred or Reg Health Information Tech-Preferred or Reg Health Information Admin-Preferred or Cert Coding Spec-Preferred or Cert Prof Coder-Preferred
Additional License(s) and Certification(s): Required Minimum Experience: Minimum 5 years of acute care inpatient and/or IVR coding experience (with Same day surgery, ED and OP diagnostic outpatient coding experiences are a plus) preferred Preferred and Must have and maintain a or higher accuracy in abstracting, code and DRG assignment while meeting productivity requirements. Required Required Minimum Skills: Computer/data entry experience. Ability to communicate with various members of the healthcare team. Ability to use EXCEL, Word and have basic computer operational knowledge. Epic and 3M experience preferred. Ability to work in a remote environment.
