Part-Time Or PRN Behavioral Health RN – Visits

On an Average Day, You Will:

• Build meaningful connections with patients and families by assessing their physical, emotional, and social needs and shaping care plans that honor the whole person.

• Take ownership of your caseload—planning, coordinating, and delivering care that makes a real difference in patients’ daily lives.

• Reassess patients regularly, making thoughtful adjustments to care plans as their needs evolve.

• Communicate openly with supervisors, physicians, and care teams about any changes in condition, ensuring patients stay safe and supported.

• Guide and empower patients, families, and caregivers to actively participate in care, creating a sense of partnership and trust.

• Share your expertise by teaching behavioral health practices, pain management, and symptom relief strategies to staff in nursing homes, hospitals, and assisted-living facilities.

• Support and mentor team members like LPNs and behavioral health aides, ensuring compassionate, high-quality care is consistently delivered.

• Keep a keen eye on patient environments, addressing any concerns to maintain safety and dignity in care.

Requirements:

• Current, unrestricted state RN licensee
• 1+ years of experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%
• Associates Degree or Bachelors Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with Homecare HomeBase is preferred

You will report to the Clinical Manager or Clinical Supervisor
