About the Role 

The Cancer Navigator at Associated Medical Professionals of NY (AMP) will work collaboratively with the Urology and Radiation Oncology Healthcare Team to plan, facilitate, track and monitor patients with Bladder and Kidney Cancer.  The CN makes appropriate referrals to meet the patient’s individual, cultural and spiritual needs by working closely with the patient and the patient’s family.  The CN will serve as a Liaison to and in collaboration with patients, physicians, disease management liaisons, and our healthcare providers to guide patients with Bladder and Kidney cancer to achieve optimal care continuity and outcomes. This is a hybrid role with some travel to our affiliates maybe required; defined by the needs of the business.

What You’ll Be Doing 

  • Implement established Bladder and Kidney cancer protocols/pathways.
  • Continue development and improvement of cancer care protocols with the Medical Director and Urology care team.
  • Serve as a resource for community support group collaboration.
  • Identify benefits and coordinate resources based upon patient’s needs, insurance coverage and community programs with the help of the Prior-Authorization department for on label infusions (i.e. Keytruda, Nivolumab, etc.)
  • Assist management in quality reporting, patient satisfaction, and treatment adherence metrics.
  • Follow through with patient’s medication treatment plan, ensuring they have received education and medication as prescribed.
  • Works closely with providers and staff to build communication, trust and a shared commitment to serving patients across all Urology divisions and Radiation Oncology.
  • Serves as an essential link between patients and all other care providers.  Facilitates appointments for consults and support services.
  • Responsible for tracking all cancer patients in all divisions to ensure patients are receiving all appropriate medical care, imaging, prescription refills, referrals, etc. and continue to be engaged in their care.
  • Identifies barriers to the plan of care and intervene as appropriate after discussion with Prostate Cancer Champion Physician.
  • Identifies patients for enrollment on clinical protocols
  • Serves as a study coordinator for clinical trials which involve utilization of clinical skills
  • Perform vital signs including 3 Lead and 12 Lead EKG
  • Perform routine Venipuncture
  • Obtain/Maintain/Discontinue peripheral IV Access
  • Infuse both FDA approved medications as well as clinical protocol drugs
  • Access Central line and perform Central Line Dressing Changes (Sterile Technique)
  • Follow all ACLS guidelines
  • Feel comfortable in working in a CODE/ Anaphylaxis/ Respiratory Arrest Situation
  • Assess and manage an Infusion reaction under the care of MD/NP
  • Follow all policies and procedures when caring for our Immuno-Oncology patients

What We Expect from You 

Preferred Skills:

  • Perform vital signs including 3 Lead and 12 Lead EKG
  • Perform routine Venipuncture.
  • Obtain peripheral IV Access
    • Assess IV site and document appropriately.
    • Able to IV push meds
    • Discontinue IV catheter.
    • Manage Infiltration/Phlebitis

  • Graduate of an accredited RN Program 
  • Current RN Licensure required 
  • Three (3) years of clinical-setting experience – Urology preferred 

Reasoning Ability  

 

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

 

Computer Skills 

 

To perform this job successfully, an individual should have thorough knowledge in computer information systems.

Salary Range: Please note that the hourly range for this position will vary based on experience level, education and geographical location.

$27.31-$33.74 / hour
