Job Summary
The Home Infusion Nurse Liaison fills an integral role in the clinical operations of the Pharmacy, working collaboratively with a multi-disciplinary team to ensure infusion therapy can be safely and effectively administered to each home-based patient. The ideal candidate will have a strong clinical background, excellent communication and customer service skills, and a commitment to providing the highest quality of care. Position responsible for the assessment, planning, implementation, and evaluation of home infusion nursing care for assigned patients, as well as providing education and support to patients and their caregivers. In addition, they will be the liaison between referral sources and the pharmacy, helping to grow the pharmacy business. This position typically reports to the Home Infusion Pharmacy Manager.RN Degree Preferred
MINIMUM QUALIFICATIONS
Education: Graduate of an accredited School of Professional Nursing; Bachelors of Nursing preferred
Licenses/Certifications:
Current State of Texas license or temporary/compact license to practice professional nursing
Certified in Basic Life Support
Infusion Nursing Certification (CRNI) preferred
Possession of a valid driver's license (if from out of state, must obtain a Texas Driver's license within 90 days after hire)
Experience / Knowledge / Skills:
