Role Description
The Population Health Manager will be instrumental in coordinating and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. The role will be an effective cross team leader and support, working side by side with providers, care teams, operations team members, and Treehouse team members.
Content areas for the Manager will include, but not be limited to, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, accurate clinical documentation, and medical cost management -- with a focus on avoidable admissions. This will require building relationships with our clinicians and center team members.
This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within an Oak Street division. This role will be aligned with a geographic division to rapidly drive results.
This role will report directly to the Regional Vice President; with a dotted line to the central Population Health team.
Core Responsibilities:
Lead strategic planning, execution and the ongoing refinement of Population Health programs in the respective region.
Work side by side with regional leaders and center care teams to support the Oak Street care model including but not limited to clinical documentation excellence, workflow optimization, and avoidable admission prevention.
Assist with creating solutions to support accurate clinical documentation and avoidable admission prevention strategies that are scalable across multiple centers.
Work closely with functional leaders in the central office to assist with strategy and program design for accurate clinical documentation and medical cost / admission efforts.
Lead the analytics, insights, and project management that drive population health execution in the region.
Be accountable for medical scribe cohort recruitment, retention, performance and engagement in the respective region to ensure clinical documentation excellence.
Accountability to key population health metrics including quality measures, utilization measures, and financial measures.
Other duties, as assigned.
We’re looking for a motivated leader with:
Minimum of 3 years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields
Strong communication skills
Ability to build the trust and confidence of a wide range of stakeholders
Leadership potential and ability to provide guidance to help team members execute on population health priorities
Demonstrated history of creative problem-solving
Strong organization and detail orientation skills
Strong skills in Excel and Google Apps
Past scribing experience is a plus, but not required
Bachelor’s degree required
US work authorization

The typical pay range for this role is:
$60,300.00 - $132,600.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
