Our Home Care Nursing department is looking to add a Registered Nurse for our western counties. As a homecare nurse, you will provide practical support in the home to patients and their families to prevent hospital readmissions. This allows patients to remain safe in their homes for a longer period of time and develop the skills to manage their chronic illness and disease as independently as possible.

As a Homecare Nurse, you will be provided with field training with the support of a preceptor as well as education on the use of homecare specific EMR. We offer tuition reimbursement as well as additional profession development provided through daily huddles, monthly Nursing Townhalls, and quarterly All-staff Townhalls. This is a PRN position to care for patients in their home in our West of I-77 Service area. You will be required to work 48 hours every 6 weeks and complete a full time orientation (M-F, 8a-5p) for the first 8 weeks.

Responsibilities:
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Performs accurate and complete admission assessments/reassessments within specified time frame.
Assesses patient for safety risks, abuse and neglect.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Assesses and documents patients' pain using appropriate pain scale and manages patients' pain.
Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family and health care team.
Maintains complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Escalates change in patient condition to patient’s physician as appropriate.

Education:
Certifications:
Current state licensure as Registered Nurse (RN).
Basic Life Support (BLS) through American Heart Association (AHA).
Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter.
Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.
