This is a part-time position supporting Fast Track Cardiology at Advocate Trinity Hospital in Chicago.

Hours: 8am-430pm

Pay Range

$54.90 - $82.35

Clinical Responsibilities:

  • Perform comprehensive assessments, establish diagnoses, and develop or adjust treatment plans tailored to patient age, acuity, and clinical condition.
  • Order, interpret, and act on diagnostic studies; prescribe pharmacologic and non-pharmacologic interventions in alignment with best practices.
  • Manage complex conditions using evidence-based guidelines, balancing clinical indications, cost-effectiveness, and risk-benefit considerations.
  • Provide health promotion, disease prevention, and patient/family education to support long-term management of heart failure.
  • Support initiatives aimed at reducing cardiac-related hospital admissions through proactive management and patient education.
  • Collaborate within an interdisciplinary team, seeking physician input or specialty referrals as appropriate; escalate care for emergent needs.
  • Maintain accurate, timely documentation in the electronic medical record and ensure compliance with coding and billing standards.
  • Facilitate coordinated care and clear communication among all care team members and community resources.
  • Perform office or hospital-based procedures within granted privileges and demonstrated competencies.

What You’ll Need to Succeed:

  • Advanced Cardiology Expertise: Strong foundation in managing heart failure and acute cardiac conditions, including medication titration, fluid management, and interpretation of cardiac diagnostics to prevent readmissions.
  • Proactive Care Approach: Ability to identify early warning signs and implement interventions that reduce hospitalizations and improve patient outcomes.
  • Critical Thinking Under Pressure: Make sound clinical decisions for complex cardiac cases, balancing evidence-based care with individualized treatment plans.
  • Collaborative Leadership: Work independently while partnering closely with cardiologists, nurses, and the interdisciplinary team to streamline care and support Fast Track protocols.
  • Patient Engagement Skills: Deliver clear, compassionate education to patients and families on disease management, lifestyle changes, and adherence strategies—key to reducing cardiac-related admissions.
  • Tech-Savvy Care Delivery: Proficiency with electronic medical records and comfort using evolving cardiac care technology, including remote monitoring tools.
  • Organizational Excellence: Ability to manage multiple patients efficiently in a fast-paced environment while maintaining accuracy and quality of care.

Licensure/Certification:

  • Active RN and APRN/APRN-FPA/APNP license(s) in state(s) of practice
  • National board certification in clinical specialty and population served
  • Active DEA registration and Illinois Controlled Substance License (prior to hire)
  • Current BLS and ACLS (or obtained within 6 months of hire)

Education:

  • Master’s Degree in Nursing

Experience:

  • Prior experience required in Cardiology.

Physical Requirements

  • Ability to stand, walk, bend, stoop, and twist throughout the workday
  • Functional speech, vision, touch, and hearing
  • Ability to lift up to 50 lbs from floor to waist, 20 lbs overhead, and carry up to 40 lbs
