About the Role
An opportunity currently exists for an experienced allied health professional/nurse to join the Health Independence Program (HIP) Central Intake team in the role of HIP Intake Clinician.
The HIP Intake Clinician will be responsible for managing referrals, including identifying client needs, and accepting referrals for the most appropriate HIP service/s, whilst working closely with the various services to ensure accurate knowledge of eligibility criteria and current wait times.
Duties include:
Triage referrals with consideration to clinical need, risk, urgency and service capacity.
Conduct initial needs identification and prioritisation of new referrals according to the HIP Guidelines.
Advocate on behalf of client as necessary and make referrals to other agencies as required.
Provide education, support and advice to referrers regarding the HIP suite of service provision, client eligibility and alternative referral options.
Provide allied health/nursing advice and support to the wider HIP Central team
Classifications for this position will range from:
Allied Health Grade 2 ($88,467 - $101,691 per annum, pro rata).
Registered Nurse Grade 3B YU11 ($102,278 per annum, pro rata).
Your Contribution
Allied Health or nursing qualification (Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) where required).
Strong ability to problem solve referrals including understanding the referrer’s request and assessing the client’s needs.
Highly developed clinical management and communication skills.
Working knowledge of the health care system and an understanding of the role of hospital based community services and of community based programs and services available to clients.
Ability to work within a high demand, dynamic environment including demonstrated ability to prioritise competing demands
Current National criminal history check, or willing to obtain

About the Department
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.
This position will be part of the HIP Central Intake Team. The key aim of HIP Central Intake Team is to provide an access point for HIP Services. Referral processing, initial needs assessment and recruitment to programs are the main functions of the HIP Central Team.
