Hourly Pay Range:
$61.80 - $95.79 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
Position Highlights:
- Position: Director, Post-Acute Services and Population Health
- Location: Evanston, IL
- Full Time/Part Time: Full time
- Hours: Monday-Friday, 8-5:30pm
- Required Travel: Yes-to all Endeavor sites
What you will do:
- Work with the Vice President, Stewardship and Physician Advisor Services, acute care management, population health leaders, program physicians and others to develop and implement an integrated post-acute care management model, focusing on populations’ care transitions from the hospital acute care setting to a variety of post-acute settings (home, SNF, acute rehab, primary care). Collaborates system-wide with north and south region leaders, physicians, community based post-acute partners and others to identify and implement innovative models and best practices, emphasizing quality and service improvements and cost reduction
- Leads and manages the fiscal efficiency of assigned areas of responsibility in the clinically integrated network
- Oversees the health equity program by ensuring alignment and impact of our Community Health Worker program to move key organizational metrics across quality and care transitions; overseeing the language services program and connecting to the quality efforts across Endeavor to ensure prioritized outcomes are equitable and sustainable
- Selects, hires, supervises and evaluates the performance of Leaders at the manager level for assigned operational units in the clinically integrated network
What you will need:
- Education:
- required: Clinical Bachelor’s degree in licensed discipline;
- required: Master’s Degree in business or a healthcare related field
- Licensure:
- required: Licensed healthcare professional in Nursing, Social Work or other appropriate clinical discipline
- Experience: Minimum eight (8) years or leadership experience in an acute or post-acute health care setting required
- Unique or Preferred Skills
- Strategic thinker with experience in a value-based care management environment; Care Management/Utilization Review experience with CMSA or ACMA certification preferred.
- Experience working with physician colleagues to build and advance clinical programs
- Expert interpersonal, oral and written communication skills; able to navigate EMR and Microsoft Office Suite of applications
- Expert leadership, change management and project management and presentation skills
- Experience in data management and using data to inform decision-making
