Essential Job Statements
Leads all aspects of the development, implementation, evaluation, and sustainability of VCUHS’s safety net strategy.
Builds healthcare partnership models with safety net partners that support VCUHS clinical strategies, shared quality goals and innovative payment mechanisms.
Partners with VCUHS transformation office and department leaders to design workflows and processes that maximize patient’s access to care.
Partners with VCU schools to develop academic/safety net partnerships that support education and training for VCU students and trainees and expands access to care for patients.
Identifies evidence-based strategies and best practices for the deployment of care models and financing mechanisms between health systems, health providers, community-based organizations, and health payers.
Explores value-based partnership models with safety net and community-based organizations and supports VCU Heath’s Managed Care team with implementation.
Manages onboarding and growth of safety net partners across VCUHS’s service areas.
Develops and leads process for outcome tracking, impact reporting, and continuous process improvement.
Reviews and reports on national and local trends in health policy and community investments related to the healthcare safety net.
Builds and maintains strong community relationships.
Supports vision and goals of VCU Health Quest 2028 Strategic Plan to Advance Health Equity.
Patient Population: Demonstrates the knowledge and skills necessary to provide equitable care appropriate to the age of the patients served on their assigned unit. Demonstrates knowledge and related competencies of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to their age, specific needs and to provide the equitable care needed as described in departmental policies and procedures. Required Education: Bachelor’s degree in Public Health, Health Administration, Health Policy or related field

Minimum Qualifications
Years and Type of Required Experience: 5+ years of project management or leadership experience in healthcare field requiring partnering with business stakeholders, project sponsors, department heads, community-based organizations, and/or other social service. Experience in managing cross-functional projects including planning and organizing work to achieve project and organizational objectives. Strong project management and/or strategic planning experience in highly matrixed healthcare organizations. Experience developing and leading community health improvement initiatives that address health disparities. Experience working with community-based organizations and diverse populations.
Other Knowledge, Skills and Abilities Required: Knowledge of Federally Qualified Health Clinics (FQHC) and Free Clinics and the services they provide to historically marginalized populations. Working knowledge of federal, state, and local policies related to safety net funding and payment mechanisms. Excellent oral and written communication skills. Data collection and analytics of patient data. Ability to work effectively with a wide range of internal and external stakeholders. Ability to manage multiple, complex projects, think strategically, and exercise independent judgement.

The Program Manager, Community Partnerships is responsible for designing and operationalizing VCU Health System’s (VCUHS) safety net strategy. The position seeks to establish an integrated safety net ecosystem with Federally Qualified Health Centers (FQHC) and Free Clinics and organizations that address health related social needs that strengthens safety net provider and community agency capacity, enhancing accessibility to care. The position partners with VCUHS department leaders, health payers, and community-based organizations to build and deploy care models that support shared quality goals and innovative payment mechanisms.
