We are currently seeking a passionate and dedicated Per Diem Medical Social Worker (MSW) to provide support and care in the comfort of patients' homes.

Location: Bend, OR
Salary: Starting at $38/ hour, DOE!

Qualifications:
- Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Master’s or doctoral degree in social work is a plus.
- Licensed as a licensed clinical social worker.
- Minimum of one (1) year of experience in healthcare, with preference given to candidates with experience in home health care.
- Excellent verbal and written communication skills, as well as strong organizational abilities.
- Current CPR certification.
- Must be a licensed driver with access to a reliable, insured vehicle in good working order.

Responsibilities:
- Assess the psychosocial status of patients in relation to their illness and environment.
- Conduct social evaluations and develop intervention plans based on assessment findings.
- Maintain accurate clinical records for all patients referred to social work services.
- Provide information and referral services to patients and families/caregivers regarding practical and environmental needs.
- Serve as a liaison between patients/families/caregivers and community agencies.
