Job Title: Hospice RN Case Manager

Location: Andover to Tewksbury (and surrounding towns)

Schedule: Monday through Friday, 8:00a-5:00p

Position Overview: As a Registered Nurse (RN) Case Manager in our Hospice service line, you will provide compassionate, high-quality care to patients in their homes, helping them navigate the challenges of serious illness. You will work collaboratively with a multidisciplinary team to ensure that each patient receives personalized care that aligns with their unique needs and wishes. This role offers a unique opportunity to build lasting relationships with patients and their families while delivering care that truly makes a difference.

Key Responsibilities:

  • Manage and coordinate care for a caseload of hospice patients, ensuring that care plans are developed, implemented, and updated according to patient needs.

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.

  • Develop and revise individualized care plans in collaboration with the patient, family, and interdisciplinary team.

  • Provide direct nursing care, including symptom management, pain control, and emotional support, to hospice patients in their homes.

  • Educate patients and families on the hospice philosophy, goals of care, and the dying process.

  • Serve as a liaison between patients, families, and healthcare providers to facilitate communication and coordinate care.

  • Monitor and document patients' progress, adjusting care plans as necessary to meet changing needs.

  • Participate in interdisciplinary team meetings to discuss and evaluate patient care plans.

  • Ensure compliance with all federal, state, and local regulations, as well as company policies and procedures.

  • Provide on-call support as needed to ensure that patients receive timely care and support.

Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in the state

  • Minimum of 1-2 years of nursing experience, preferably in hospice, palliative care, or home health.

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients and families.

  • Compassionate, empathetic, and dedicated to providing high-quality care to patients with serious illness.

  • Ability to work independently and as part of a multidisciplinary team.

  • Valid driver’s license and reliable transportation to travel within the designated coverage area.
