Job Description:

SCOPE: Coordinates patient care requests—by phone and through Navigating Care or other future portal systems—for all providers-- and serves as a resource to clinic staff. Responds to pharmacy calls and manages medication refill requests. Coordinates Provider orders, including home health, hospice, and hospital services. Serves as a liaison between physicians, patients, and external facilities. Provides back-up support for clinic and intake staff. Performs other duties as requested by physician/provider or supervisor.

ESSENTIAL FUNCTIONS:

• Triage patient calls                                                             
• Responds to Navigating Care Portal messages                     
• Perform Nursing Duties                                                      
• Coordinate patient care and documents                             
• Serves as resource for all staff                                           
• Other duties as assigned            

QUALIFICATIONS:                                       

1. High School Diploma (CCMA), Nursing Diploma (LPN) required

2. LPN licensure or CCMA certification required; CPR required

3. 2 years experience required, Oncology experience preferred
