Registered Nurse (RN) Homecare – West of I77 Service Area
Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. Cleveland Clinic Homecare currently serves 14 counties in the greater Cleveland and Akron areas. Divided into 4 teams (East1, East2, West and South), each team is then further divided into smaller geographical areas where each caregiver is assigned to a smaller service area within the team. The caregiver in this role will serve patients in the West area (areas west of I-77). The exact area you will work in will be discussed during the interview with the hiring manager.
As a Home Health Registered Nurse, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. This position is eligible for student loan repayment.
A caregiver in this position works Monday – Friday starting around 7:00AM/8:30AM and is required to complete 52 units every two weeks. A unit is the value of a task in the homecare compensation model/productivity standard and allows for flexibility in day-to-day responsibilities as productivity is not counted per day, but rather per week. Caregivers in this role are required to work one weekend every four weeks, two holidays per year and one on-call shift per week.
A caregiver who excels in this role will:
Provide direct nursing care.
Perform accurate and complete admission assessments and reassessments within a specified time frame.
Assess patients for safety risks, abuse and neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess, document, prioritize and manage patients’ pain.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of professional nursing.
Current state licensure as a Registered Nurse (RN).
One year of clinical experience as an RN in an acute med/surg setting.
Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred qualifications for the ideal future caregiver include:
Bachelor of science in nursing (BSN). A BSN is required within three years of hire date if a diploma or associate’s degree of nursing (ADN) has been obtained.
Infusion therapy experience.
Familiarity with complex medical treatments.
Experience working in homecare, med/surg, ICU and/or ED areas.

Physical Requirements:
Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
Must be able to read, write and communicate with patients.
Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
Must present a professional appearance at all times and attend staff and agency meetings, as requested.
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
