Job Description

The Patient Navigator manages a variety of office functions and is key to smooth operations of the Division. The primary function of this position will be to coordinate health care services, in partnership with patients and care team members, across the continuum of care in order to maximize and improve patient health outcomes.

The core functions of a Patient Navigator includes: assisting patients and families with understanding and adhering to treatment plans; assisting patients in accessing health services; ensuring barriers to obtaining care and/or social services are addressed; linking patients and families to community agencies and services; and collaborating with the care team, regarding transitions to care.

Essential Job Functions:

  • Assist with patient appointments as necessary; including appointment reminders for initial diagnostic and/or follow up appointments
  • Assist patients with obtaining necessary medical records (e.g. films, pathology reports, etc) for appointments, assembles new patient charts, maintains and files existing patient charts, and spot checks charts for data completeness and signatures.
  • Coordinates/schedules ancillary testing with other hospital departments
  • Arranges for patient transportation as needed
  • Re-engage patients that have been lost to care
  • Regularly collaborate, coordinate, and communicate with care team members(s) to resolve outstanding patient care related items (i.e. care gaps; outstanding referrals; completion of forms)
  • Completes referrals to appropriate support services and discharge planning as needed.
  • Partners with team members and division leadership to improve patient compliance with health care recommendations
  • Addresses the psychosocial/spiritual/developmental/cultural needs of the patient and family throughout the continuum of care.
  • Coordinates and links each patient to available community resources, including medical care, medication coverage, and other health care required services
  • Provides access and coordination to social support services by creating and implementing an individual, multi – session, time – limited intervention to connect patients with recent diagnosis and/or positive test results to primary medical care
  • Helps patients problem-solve their issues and grievances and will provide condition – specific counselling and referral services, as needed
  • Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.
  • Coordinates and manages defined projects consistent with the current, externally funded project plan and future projects.
  • Contributes to the overall program leadership through planning, interdisciplinary communication and collaboration.
  • Facilitates transition to next phase of care by serving as a liaison between settings and to help with the development of a step – by – step plan to accomplish an approved session plan
  • Works with nurse case managers and care coordinators to improve patient compliance with health care recommendations
  • Meets and/or speak with patients to facilitate understanding of their discharge and deliver all aspects of linkage services including referral, provisional eligibility determination, assessment, and client evaluation
  • As part of the transition team/navigator role, you will be expected to attend team meetings and provide feedback as to how to improve program and outreach efforts
  • Other job functions as assigned.

    Minimum Qualifications:

    • High School Diploma required, bachelor’s degree in Human Services strongly preferred.
    • Minimum of 2+ years of office administration experience, preferably in a medical setting
    • Knowledge of medical terminology preferred
    • Strong working knowledge and skills in Microsoft applications.
    • Ability to take initiative and work independently
    • Good customer, interpersonal and communication skills, both orally and in writing
    • Organizational skills: Planning and managing projects; excellent attention to detail; ability to establish priorities and react promptly to a large variety of requests and needs, sometimes on short notice.
    • Strong analytical and data management skills and the ability to exercise sound judgment, take initiative, and make diligent, thoughtful, independent, and timely decisions
    • Excellent negotiation skills
    • Excellent rapport-building and customer service skills to build and maintain positive relations internally and externally
    • Proven ability to work with groups across different cultures and different income levels

    Education

    Bachelor's Degree, High School Diploma/GED (Required)

    Pay Range

    $46,280.00-$75,670.40 Salary
