Job Description Summary:

Responsible for the individualized management of hospital and health care services necessary and appropriate to the provision of high quality client care within fiscally expedient time frames. The case manager works with acutely ill and/or high risk clients from newborn to elderly. A hospital-based case manager coordinates and performs comprehensive patient care review, which includes the integration of Utilization Review, Quality Assurance, Risk Management, Infections Control and Discharge Planning functions. As an advanced nurse clinician, the case manager combines the role of clinician, collaborator, consultant, and educator to accomplish desired clinical outcomes. The nursing process (assessment, planning, implementation, and evaluation) and management process (planning, organizing, directing, and controlling) provide the framework for decision making within this role. Monitors compliance to clinical pathways. The community based case manager is responsible for developing collaborative, outcome oriented plans of care for each client. The community case manager acts as a patient advocate, providing mechanisms and support to empower the patient to make responsible healthcare decisions.

Minimum Qualifications:

Bachelor's Degree (Required)CPR - Cardiopulmonary Resuscitation - American Red CrossMust be a graduate of an accredited school of nursing. Must be currently licensed as a Registered Nurse in the State of Ohio and have a current state driver's license. Driving record must be acceptable to the Hospital's current liability carrier for all Community Case Managers. Must have the knowledge and skills that would generally be acquired through three to five years of clinical Experience . Community or Home Health Care nursing Experience for the Community Case Manager. Work requires the ability to analyze and solve complex problems requiring the use of basic scientific clinical, or technical principles plus in-depth, Experience -based knowledge. Work requires excellent communication skills in order to interact in a professional manner with patients, administration, visitors and physicians concerning immediate case issues, policies and procedures, committees, training, etc. Knowledge, skill and ability to provide care for neonatal, pediatric, adolescent, adult and geriatric patient population. Work is not completely standardized. Assignments are often received in the form of results expected, due dates and general procedures to follow. Ability to maintain confidentiality.
