Our St Vincent's Virtual & Home program is growing, providing the opportunity for more people to receive high quality clinical care at home. Integrated with the St Vincent’s hospital and community care network we are a multidisciplinary team of physicians, allied health, nursing and personal care workers supporting individuals to leave hospital sooner, remain in their homes and achieve their goals. We are an experienced, supportive and dynamic team committed to the growth of quality care within the community.
About the Role
Location: Brisbane, Sydney or Melbourne
Weekly Hours: 0.6 EFT 22.9 hours per week
Join St Vincent’s, a leading non-profit health organisation, as a Care Coordinator. In this role, you will support client journeys, manage referrals, oversee digital care platforms, and coordinate care. Key responsibilities include client onboarding, clinical escalation management, and collaboration with team members and external service providers. We are seeking a qualified allied health or nursing professional with orthopedic, rehabilitation or community experience and strong communication skills.
