$21.85 - $32.80The Care Coordination Assistant plays a key role in supporting safe, timely, and effective patient discharges by collaborating with Care Coordination staff, medical center teams, and community partners. This position assists professional staff with discharge planning activities, facilitates the exchange of information across care settings, and helps ensure patients transition smoothly to the next level of care. The ideal candidate is organized, detail‑oriented, customer‑focused, and comfortable working in a fast‑paced healthcare environment with frequent interruptions.
Essential Functions
Assist with the implementation of discharge plans by supporting discharge services, including preparing discharge packets, providing current community resource lists, initiating FL2s and PASRRs as needed, and researching resources to meet identified discharge needs.
Arrange concrete discharge services at the direction of the Case Manager and/or Social Worker, including durable medical equipment (DME), home health services, and transportation (ambulances, taxis, etc.).
Support case managers and social workers with assisted living facility (ALF) and skilled nursing facility (SNF) placement arrangements.
Contribute to efficient and effective departmental workflow through assigned responsibilities such as maintaining the clothing closet, assisting with placement processes supporting Emergency Department staff, and assisting with computer hardware and software needs.
Ensure timely delivery of Medicare Important Message (IM) letters to all Medicare and managed Medicare patients within two days of discharge.
Facilitate the smooth exchange of patient care and discharge information by communicating effectively with Care Coordination staff, other medical center departments, and outside agencies.
Assist with pulling reports as needed for CM/leadership needs, filing, data entry, and general clerical duties as needed; prioritize and organize work to ensure timely submission of reports requested by leadership.
Assist with audits of case management services and compile audit findings to support evaluation of service quality and effectiveness.
Perform reception duties by answering phones, responding to questions, and assisting patients, families, and visitors in accordance with departmental standards.
Demonstrate strong customer service skills while managing high volumes of phone calls, in‑person inquiries, and frequent interruptions.
Education and Experience
High school diploma or GED required.
Associate’s degree in a health‑related field preferred.
Two years of office experience preferred.
Excellent oral, written, and interpersonal communication skills.
Strong attention to detail and ability to organize and prioritize multiple work tasks.
Ability to work collaboratively with interdisciplinary team members to support patient care needs.
Proficiency with data entry and standard office equipment; experience with Windows, Excel, and Outlook preferred.
Ability to interact with the public in a friendly, professional, and helpful manner in a busy healthcare
