Position Summary
This is a telework position with up to 50-75% travel in Northern Illinois, with 2-3 days being onsite at a facility in Rockford, IL.

Standard business hours Monday-Friday.

The Clinical Case Manager BH coordinates all case management activities with members to evaluate behavioral health needs and to facilitate the overall wellness of members. Develops strategy to address issues to outcomes and opportunities to enhance member's overall wellness through integration. Instructs programs and procedures in compliance with network management and clinical coverage policies. The Clinical Case Manager BH will have onsite presence at a Residential Recovery Treatment Center in Rockford, IL to coordinate with family, clinical services, treatment team and supervisors on the development, planning and implantation of therapeutic services as they relate to the individual child’s behavioral and emotional health.

Essential Duties and Responsibilities:

• Care Planning and coordination of needs

• Provides services consistent with the guiding principles of the agency’s philosophy of strength based, family focused, needs driven and culturally competent care.

• Completes assessments, formulate, and implement an overall comprehensive care plan for each child and family, consistent with the needs of the child and family team and the plan.

• Gather information and develop a comprehensive care plan that addresses risk factors, psychosocial history, substance abuse history, mental health history, community functioning, performance outcomes, strengths, challenges, goals, interventions, and discharge planning according to the programs mission.

• Facilitates and coordinates Family Support Meetings.

• Identify potential problems and crisis areas, prioritize according to seriousness, and create an effective and well specified safety plan.

• Collaborate with family, clinical team members, county workers, external providers, and natural supports for case conceptualization, assessment, and treatment planning

Required Qualifications

-One of the following active and good standing licenses is required for Illinois: LCPC, LCSW, or LMFT
-Minimum 2 years of clinical experience.

-Minimum 2 years of experience working with children.

-Willing and able to travel up to 50-75% of their time to meet members face to face throughout Northern Illinois.

-Willing to be onsite at a Residential Recovery Treatment Center in Rockford, IL at least 2-3 times a week.
-Reliable transportation required; Mileage is reimbursed per our company expense reimbursement policy.

Education

Masters Degree in Social Work, Counseling or any other related field is required.

Pay Range

The typical pay range for this role is:

$63,200.80 - $136,600.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
