Crossroads is a leading addiction treatment provider of outpatient medication-assisted treatment (MAT). We treat patients with opioid use disorder (OUD) using medications such as methadone and suboxone/ buprenorphine. Day in the Life of a Patient Navigator

  • Greeting patients and answering clinic phones.

  • Assist patients in understanding their treatment options, and the resources.

  • Serve as an essential link between patients and all other care.

  • Facilitate appointments for consults and support services within established service standards, including social workers, physician, etc.

  • Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.

  • Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management.

  • Work with staff and others to collect data, track outcomes, and support strategic planning processes.

  • Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, social services.

  • Assist Center in stocking supplies and Patient Ambassador team. ​

  • Other duties as assigned.

Education and Experience requirements:

Must hold one of the following credentials/experiences:

  • RN, LPN, or BSW

  • Bachelor’s Degree WITH at least 2 years of lived addiction experience.

  • Associate's degree or CMA WITH 4 years of lived addiction experience.

Hours, Schedule, and Travel (if applicable)

Mileage reimbursement: Crossroads matches the current IRS mileage reimbursement rate.

  • Monday - Belvidere; 8:30am-12:30pm

  • Tuesday - Hamburg 2:30pm-7:15pm

  • Wednesday - Stanhope 10:00am-2:00pm

  • Thursday - Belvidere 11:30am-5:00pm

  • Sunday - Stanhope 9:00am-1:00pm
