Job Summary:
The LPN Intake Specialist is responsible for intake functions focused on arranging/monitoring hospice ancillary services needed by the Hospice patient for admission at home or the IPU setting. Those services could include pharmacy enrollment, DME ordering/verifying need and arrival of equipment to patient's home setting, arranging transportation of the patient to/from home to/from IPU setting, arranging, and ordering IV, Respiratory equipment for the Home Hospice and IPU Patient. Core Responsibilities and Essential Functions:
Customer Service (Processes via phone, email or fax all vendor orders for Hospice care. Communicates with customers, handles service problems politely and efficiently, always available for customers, follows procedure to solve customer problems, understands company products and services, maintains pleasant and professional image).
* Answer incoming calls and provide each caller, referral source with ease of entry into Wellstar Hospice care.
* Process each medication, transport and DME request with accuracy
* Ensure positive working relationships between Wellstar Hospice departments and patients and families, and all other referral sources.
* Communicate with Liaisons, patients, and families to explain DME information etc. as appropriate.
Personal Organization (Keeps information organized and accessible, maintains clean/functional workspace, works systematically/efficiently, manages time well)
Vendor order processing:
* Process orders in a timely manner and efficient manner.
* Check orders for completeness and qualifying criteria.
* Obtain missing or incomplete information from referral source or liaisons
Other duties
* Work with clinicians, as needed
* Work with QA to obtain timely documentation
* Complete other documentation, as necessary
Performs other duties as assigned
Complies with all WellStar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
- High School Diploma General or GED General
- Other / Certificate Nursing
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 2 years Health care related experience preferably in Hospice Required
Required Minimum Skills:
Professional, independent, self-directed.
Strong organizational skills, ability to multi-task.
Excellent communication skills.
Computer proficiency a must along with excellent phone communication skills, customer service oriented.
