JOB DESCRIPTION:

SUMMARY OF POSITION

The Intake Coordinator is the point of contact for potential clients for the Steven A. Cohen Military Family Clinic.  This position is responsible for managing and executing intake calls and face-to-face screenings with potential clients, with focus being on post 9/11 veterans and their families in the community. The Intake Coordinator will additionally administer and complete appropriate initial paperwork, psychometric screeners, and health record documentation to support clinical staff for ongoing treatment of the client.  The intake coordinator will work with a multi-disciplinary team of clinic staff for case assignments, consultations and care coordination.

 

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Conduct telephonic triage and schedule initial intakes with Veterans and/or their family members.
  • Document detailed intakes in the electronic health records (EHR) system; maintain comprehensive client referral tracking system; prepare reports as requested by the Clinic Director.
  • Meet regularly with the Clinic Director and clinic multi-disciplinary team to coordinate case assignments and report referral-to-admission conversion rates, screener completion rates, etc.
  • Provide ongoing program evaluation to the Clinic Director for continuous growth and quality improvement efforts.
  • Refer, as appropriate, to local community resources and provide warm hand-offs for Veterans and families needing external referrals.

  • Capacity to remain calm and maintain self-control in the midst of difficult circumstances and emergencies. Ability to respond in a professional manner in all situations.
  • Proficiency with Word, Excel, Publisher, Access and PowerPoint preferred.
  • Capability to work independently, manage work priorities and to evaluate and create solutions to work related problems.
  • Ability to think analytically and make appropriate treatment recommendations.
  • Skill in negotiating clients down from stressful situations and maintain composure during potential crisis situations.
  • Ability to assist clients in navigating treatment options. Knowledge of community resources.
  • Knowledge of mental illness and mental health diagnoses required.
  • Knowledge of various treatment modalities and methods preferred.

QUALIFICATIONS

         

Education Level              

Bachelor's degree in Social Service field required.

Years of Experience       

  • 3+ years post-Masters intake coordination and/or case management preferred.
  • Prior experience in serving the military community and/or veteran administrative organizations required. Military background preferred.

Certification/Licensure 

Driver’s license with clear record required. Must have daily use of a vehicle.

$24.38--$34.18
