About the HOPE Heart Failure Clinic
The Corewell Health Heart OPtimization & Evaluation (HOPE) Clinic is a multidisciplinary clinic devoted to the care of patients with advanced heart failure. The multidisciplinary team consists of physicians, Advanced Practice Providers, RNs, medical assistants, pharmacy technician, pharmacist, and medical social workers. Additionally, HOPE offers timely referral access to a dietician, cardiology trained psychologist and Palliative/Hospice teams. Many advanced heart failure patients have cardiac implanted devices. HOPE promotes the use of advanced device technologies utilizing artificial intelligence to help predict when a patient may be headed toward fluid overload and decompensation. The HOPE RN team collaborates with the device clinic to aid in device monitoring and management. Despite the advanced care options offered, a subset of HOPE patients will transfer to the Ventricular Assist Device (VAD)/Heart Transplant clinic if appropriate for those services. Most of the patients will remain with the HOPE clinic for the remainder of their life. HOPE disciplines work closely together to care for this very ill and vulnerable population.
To optimize patient outcomes, HOPE offers an enhanced Guideline Directed Medication Titration (GDMT) service. The GDMT service improves heart failure outcomes by utilizing a pharmacist to rapidly titrate GDMT medications as well as assess and monitor patient’s tolerance during titration.
The role of the RN is critical to the care of these patients. RNs teach patients to self-manage their disease, triage care to reduce hospitalizations & ER visits and promote the best quality of life. The RN’s main responsibilities consist of telephone triage, patient teaching, administering intravenous diuretics and iron infusions, performing blood draws and analysis for Point of Care blood tests. Ideal candidate will be an RN possessing Cardiology, ICU, ED, or Heart Failure experience
This RN will be assisting with clinic patients who might be clinically in need of monitoring and clinical support until transferred to an ED and is required to be ACLS trained
There will always be a Provider with this Nurse
Plan to work in Big Rapids Mini HOPE clinic:
–Starting 1 Day per Week On-Site until we move a couple more providers to work in Big Rapids
–Ultimately will be working 2 8-Hour Days after more Providers are shifted there
–When we phase up - It will be 2 Days (Tuesday and Thursdays) every Week On-Site, and every other Friday will be Remote work from home
