As a Case Manager, you will apply your clinical expertise, critical thinking, and collaborative spirit to guide patients and their care teams toward the highest-quality, most cost-effective outcomes. In this role, you’ll advocate for patients, coordinate complex services, and work closely with a multidisciplinary team dedicated to exceptional care.
Essential Functions
- Utilize the nursing process to develop, implement, and evaluate case management plans for defined patient populations.
- Serve as a liaison between patients, families, physicians, nurses, and interdisciplinary care teams to promote continuity of care and effective communication.
- Facilitate optimal patient outcomes through advocacy, education, and evidence-based decision-making.
- Analyze patient care delivery using a scientific approach to identify opportunities for improvement in quality, patient experience, and cost efficiency.
- Coordinate appropriate healthcare services across settings, ensuring timely progression of care and discharge planning.
- Collaborate daily with the case management team to align on patient needs, barriers, and expected outcomes.
- Maintain availability for patient care needs via pager, digital phone, or email when not physically on campus.
- Provide consultation and education to staff and patients regarding case management processes and resources.
Qualifications
Education
- Graduate of an accredited School of Registered Professional Nursing required
- Bachelor of Science in Nursing (BSN) preferred
- Master of Science in Nursing (MSN) encouraged
Experience
- Minimum of five (5) years of nursing experience caring for the assigned patient population
- Experience in case management and utilization review strongly encouraged
Licensure / Certification
- Current Registered Nurse (RN) license in the applicable state based on job location
- Case Management certification (e.g., CCM, ACM) encouraged
Shift / Schedule
- Full-time, Monday–Friday
- On-call availability required via pager, digital phone, or email when off campus
