The Telehealth RN, serves as a patient care liaison coordinating services throughout the continuum of care to improve quality, reduce avoidable readmissions and emergency visits and to reduce health care costs. The Telehealth RN will manage patients remotely, coordinating care and contacting physicians as appropriate. They are an integral part of preventing re-hospitalizations and Emergency Room visits for patients, facilitating care in the community through communication with physician offices, community resources, patients and families. Utilizes innovative workflows to enhance patient care and wellness. Collaborates with healthcare providers to ensure the best outcomes for the patient and provide coordination and education to all those involved in the care of the patient. Performs other duties as assigned.
This function is in accordance with the Michigan Public health Code Act 368 if 1078, Article 15 (Health Occupations), Part 172 (Nursing), and the ANA Registered Nurse Standards and the Holland Hospital Professional Nursing Practice Handbook.

Pay Range:  $32.79 - $49.18/hour

Employment Type: Part Time, 40 hours every 2 weeks

Requirements:  Graduate of accredited Registered Nursing Program required, Current Michigan RN License (or completed by end of orientation) required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required

Telehealth Patient Oversight

  • Performs daily clinical oversight of all telehealth patients, including monitoring trends, reporting to physicians and coordinating follow up care as needed.
  • Advocates for interventions to prevent re-hospitalizations and Emergency Room visits. Identifies and helps facilitate identification of telehealth appropriate patients.
  • Educates patients and internal/external stakeholders about telehealth services.

Planning and Coordination

  • Collaborates with an interdisciplinary team including but not limited to Care Management, Readmission Prevention, pharmacy, Home Health, and Physician Practices to maximize clinical quality outcomes.
  • Serves as a liaison for case updates and collaborates with post-acute providers

Assessment and Patient Care

  • Notify provider offices of Telehealth enrollment. Communication may vary based on the provider office, via fax, EMR, or phone call. Be aware of the appropriate point of contact for each provider group.
  • Provide telephonic assessment with patient including symptom specific related questions pertinent to remote patient monitoring data and vital sign instability, collaborate with patient/families/providers to establish follow-up visits, assess for potential needs, identify readmission risks, and resolve issues.
  • Triage level of contact necessary based on data coming through from remote patient monitoring and patient stability. Works collaboratively with home health staff and delegates tasks and provides general supervision to telehealth assistants in the provision of patient care.  Provides advice and guidance to non-RN Home Health Staff on recommended actions in the event of vital sign concerns.
  • Complete documentation in RPM EMR and in other EMRs for mutual stakeholders for communication of vital sign concerns, next labs due, follow-up appointment, cardiac rehab hand-off, discharge plan etc.

Programmatic Improvement

  • Participates in investigations as to the root cause of readmissions.
  • Aware of all processes and offers suggestions and plans for process improvements.
