Across NewYork-Presbyterian/Morgan Stanley Children's Hospital, Care Coordinators are the guardians of continuity of care; they are the skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes.

As a Care Manager for our 75 bed Neonatial ICU you will be responsible for working with a interdisciplinary team and a Social Worker dyad to implement a plan of care and follow the patient to check that all aspects of the care plan are met to facilitate a safe disposition plan, transitioning to home or the next level of care. You will help arrange for peer to peer with insurance companies denying plan. You will work alongside DME vendors to ensure all equipment needed is provided as well as teaching from both staff and vendor. Candidate must have strong communicating skills. You will perform prior authorizations for any equipment or medications ensuring they are present at the time of discharge. You are expected to communicate with both interdisciplinary team as well as Social Worker dyad in preparation for transition of care.

This is a Monday to Friday position with weekend and holiday rotations.

Preferred Criteria

  • 2 years' of case management/care coordination experience in an inpatient Pediatric or OB setting

  • 1 year of leadership experience in a healthcare setting

  • Master's Degree in healthcare related field 

Required Criteria

  • Bachelors degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 to 5 years of health care experience in an acute care setting (Peds experience essential)

  • Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

Salary Range:

$115,000-$162,500/Annual
