Summary:

Provides patient centered customer service to internal and external clients. Serves as a liaison between the pharmacy, payors, patients and providers. In addition, coordinates care to ensure patient intake, prescription processing, resolution of third party rejections, medication orders and delivery process.

Responsibilities:

Performs patient intake, benefits investigation, obtains copay assistance and provides link to patient assistance or triagesto other pharmacies or servicesCollaborates with clinical staff to facilitate patient and/or caregiver requestsCoordinates delivery of medications in a timely manner to patient's preferred location (bedside, home, outpatientpharmacy) to ensure patients receive treatment in a timely mannerCollects, documents and reviews all needed information to ensure timely coordination of careCoordinates care with clinical staff with any patient side effect and/or request from providers, patient or caregiverSupports staff training, transitioning and onboardingCommunicates effectively in person, over the phone and in writing to provide customer service to the internal and externalcustomers (patient, caregiver, physician, pharmacist) and liaisons between patient and providers for medication needs,including patient information, prescription processing, third party rejection resolution, delivery, storage, supplies, refills,education materialsMaintains compliance with department workflow assignments and metricsPerforms activities related to pharmacy technician scope of practice
